SIGHT FIRST!
Purpose:
The Lions Club primary goal has always been Sight First. Our priority is given to children attending school and the elderly (depending on degree of need) on the Navajo reservation. Applications will be determined upon availability of funds and requires an in-kind payment of $15.00 per applicant.

Guidelines for the Window Rock / Fort Defiance Lions Club Eyeglass Program
- Once again the Window Rock / Fort Defiance Lions Club is working to fulfill our commitment to “Sight First”.
- This eyeglass program is open to everyone who demonstrates a need, but priority is given to the student and the elderly first.
- Because of limited funds, all grants are dependant upon availability of funds and require an in-kind contribution of $15.00. The grantee will only be served once per year.
- The application form is used to help the eyeglass committee assess need with priority given to the most severe sight loss. Financial need is also assessed but is secondary.
1. The applicant makes the initial request to a member of the Lions Club or in the case of a student; a teacher thru the School Health person who will initiate the request. The Lions Club Member or the School Health Person will provide an Application Form to the applicant.

Because of limited funding we are able to serve only five individuals per month. Each application will be evaluated to determine need. Those with the most need take priority.
Each applicant will complete and sign a club application form, which can be obtained from any club member or through a school district's School Nurse or Health Aide.
The person taking the application will then collect the in-kind contribution of $15.00 in the form of a money order made out to Lions Club. (No cash will be accepted) Then turn it in to the Lions Club Eyeglass Committee by the second Monday of the month. Upon approval of an application a purchase order number will be issued to the eyeglass provider.
